Enrollment

All information provided on the enrollment form and/or requests for student records are for informational purposes and/or cohort placement. Enrollment at Academy of Tucson Schools is open. The Academy of Tucson does not and shall not discriminate on the basis of race, color, religion (creed), gender, gender expression, age, national origin (ancestry), disability, marital status, sexual orientation, or military status, in any of its activities or operations.

Please complete the online enrollment form below. If you prefer, you may click the links to the right to download and print a paper enrollment form and submit to your school of choice.  

 Additionally, you will need to fill out the Arizona Residency form and email the completed form to [email protected].. 

Thank you for completing the enrollment form. Our schools will be in contact soon and will need the following documents to complete the enrollment process (these may be provided within 30 days):
  • A certified copy of the student’s birth certificate OR other reliable proof of age and identity (including baptismal certificate, social security number application, or original school registration records and an affidavit explaining the inability to provide a copy of the birth certificate) 
  • Proof of Arizona residency 
  • Immunization records or other written certification or documentation 
Answers provided or missing information does not negatively impact enrollment or enrollment status.
